Field Trips

The following forms must be completed before any field trip:

Leader: A Proposal for off-campus fieldwork that includes a Pre-trip Evaluation Worksheet, the Leader portion of the University Field Activities Notification, and Travel Pre-Authorization Form.

Student: the Student portion of the University Field Activities Notification, and Release of Liability (University Release Agreement)
